Physical Examinations

Please register for the physical check-up below. If you belong to a Japanese company, Please write down your company’s name. Please complete pages of a checkup course, date/time, and health questionnaire.

Once registration is completed, the User ID / the password will be sent to you. Please keep your User ID/password information as a record.

Annual Health Checkup Courses

Available days：

Monday to Friday : 9AM to 5PM (It depends on the courses)

Saturday: 10AM to 3PM (It depends on the courses)

Duration of examinations：

2-4 hours (depending on the courses)

Payment：

We accept all major credit cards, personal checks, and cash payments. You can pay at the front desk when your examination is completed. If your employer will cover for your services, we will bill your employer later. Corporate covered physical exams require pre-authorization with the authorized person’s name and signature on a specified form.

Insurance：

If you would like to use your insurance, we can bill your insurance company. Please verify plan coverage details in advance as you will be responsible for services that are not covered.

Important Information for Your Examinations：

Do not drink (except for water) or eat anything 12 hours before your appointment.
If you plan to take Upper Gastrointestinal X-ray, Abdominal Ultrasound, and Gastroscopy, do not drink or eat anything including water 12 hours prior to your appointment.
If you have medications you have to take regularly, please take them with minimum water.
No smoking until the end of all examinations
Please take adequate rest and no alcohol before the examination.
If you are taking medications for treatment or are on a diet for Diabetes, please notify our staff before your appointment.
Do not use body lotion/oil/powder or perfume.
If you wear glasses or contact lenses, please bring them with you to the examination.
If you need to change your appointment or exam items, please notify us as soon as possible.
If you cancel/reschedule your appointment, please give us a call at least 24 hours in advance. A cancellation fee of $50.00 will apply if cancellation is made less than 24 hours.

<For Women>

Please advise us if you are or may be pregnant.
Pap smear test and urinalysis cannot be performed if you are on your period.
Screening mammography for women at 40 years old or older without any history of breast cancer and/or other breast diseases.
Pregnant women and women with breastfeeding cannot take screening mammography. If you have symptoms, please contact your doctor for guidance on further testing.

<Cancellation, Rescheduling, and No Show Policy>

If you must cancel or reschedule an appointment, please call us at least 24-hour (if your appointment is on Monday, you need to call us 48-hour prior to your appointment). Your appointment which is canceled or rescheduled with less than 24-hour (48-hour for the Monday appointment) notification will result in a $50 fee per person. If you no call/no show, you will have to pay 50% of the physical check-up cost. Ex. If your check-up course $500, you will need to pay $250.

<Child Health Check-Up / School Forms>
Children do not need to register for the annual checkup.
Please contact us to inquiry@jmedical.com and let us know your child’s name, date of birth, and appointment time.

<Check-Up Results>

We will send your checkup result about 8 weeks after your examinations (It may take a longer time for

certain test results) to your home address. We also provide express service (result within 1 week for extra $99)
